~ If you are experiencing symptoms of being ill you are required to cancel your appointment as soon as possible.
~IF YOU HAVE BEEN EXPOSED TO SOMEONE WHO HAS HAD A POSITIVE COVID TEST IN THE 2 WEEKS PRIOR TO YOUR APPOINTMENT, YOU WILL NEED TO RESCHEDULE YOUR SERVICES.
~IF YOU HAVE HAD A COVID TEST, REGARDLESS OF THE RESULTS, YOU MUST WAIT TWO WEEKS TO COME IN FOR AN APPOINTMENT.
~ MASKS ARE MANDATORY to come in the studio, if you do not have one I can provide a disposable one for you. You will be asked to sanitize your hands.
~ Every guest will need to sign the waiver below to be provided a service.

By filling out the form below and submitting, you agree to comply with the written instructions and protocols above and agree that you are at the studio at your own risk, releasing Crystal Chapman Hair Studio from any liability relating to COVID-19. Failure to comply with these written instructions or verbal instructions from staff may result in forfeiture of your services.
